The IIT ISM Health Center App provides a convenient way for students and staff to manage their healthcare needs on campus. Users can book doctor appointments, access and download medical prescriptions. Designed for secure and efficient service, the application ensures reliable and protected healthcare management anytime, anywhere.

I hereby give my consent to voluntarily participate in the Digital Health Application being implemented at the Health Centre of IIT (ISM) Dhanbad. The initiative, funded by ICSSR under VISION VIKSIT BHARAT@2047 in collaboration with Healthinnovation Toolbox under the project “Future-ready Hospital: Strategic Adoption of Industry 4.0 Technologies”, aims to streamline patient care, e-prescription, data management, and medicine dispensing.

I understand and acknowledge that:

• The application is deployed on a secure IIT (ISM) local environment.
• My data (such as name, contact details, and registration-related information) will be collected solely for testing, validation, and improvement purposes and will not be used for any commercial or external purpose.
• All data collected during this phase will be handled in compliance with the Digital Personal Data Protection (DPDP) Act, 2023.
• Participation in this pilot is completely voluntary, and I may withdraw my consent at any time.

User data will be securely stored within the IIT (ISM) authorized server environment only. No personal data will be shared with third parties. Data will be retained only for the duration required for testing and validation of the application.

Once the testing phase is completed or upon request, user data will be permanently deleted from all systems.
